Ranking #1 on Google Is a Coin Flip for Getting Cited by AI
AI Overview citation is related to organic ranking, but it’s a distinct, weaker mechanism than most people assume. Which means a page that doesn’t rank well can still get quoted by AI.

For twenty years, the goal for search inclusion was simple: rank at the top of Google. With AI search in the mix, this goal looks different. Understanding this is important for dental marketing, because it changes which pages are worth investing in to support your online visibility with patients searching for a dentist online.
What the research says
Ahrefs analyzed 1.9 million citations drawn from a million AI Overviews. Most of the cited pages, about 76% of them, did rank in the top 10 organically. So ranking does still matter to some degree, but the relationship is looser than it looks.
14% of cited pages didn’t appear in Google’s top 100 at all. This means that the correlation between #1 organic Google rankings and AI citations is a coin flip at best. Being cited and ranking well are related mechanisms, but they’re not the same mechanism.
Why this is good news for your practice
Traditional SEO punished you for not being at the top. No top placement, no visibility. The AI citation model is more forgiving, because your page’s content can earn you a citation by being the best, most specific answer to a particular question, even if your traditional SEO rankings say you’re invisible.
For a dental practice, that means a well-built website page answering a specific patient question can get quoted by AI without first winning a ranking war it might never win.
The limits of this view
While the research Ahrefs did used a very large sample and a disclosed methodology, it has not yet been cross-verified by a second independent organization, and it doesn’t consider dental or local queries specifically. As our AEO/GEO team explores what’s needed for dental practices to win AI citations, we’re considering this information as a good sign, but treating it as something that we need to confirm first.
What to do with this information
Stop gating your content decisions on SEO ranking alone. A page doesn’t need to reach the top of Google before it’s worth optimizing more, because usefulness on a precise question is its own path to being cited, ranking or not.
If you want to find which specific questions your patients are asking that your site could own, that’s the kind of thing we can work through with you. Book a call, and we’ll dig into it.
